Monthly out-of-pocket cost in dollars, before the Bridge. Zepbound self-pay varies by dose: the 2.5 mg starter dose runs about $349/month, 5 mg about $399, and all higher maintenance doses about $449 — most people on maintenance therapy pay the $449 rate.

You could save through December 2027. But the Bridge only helps if your doctor routes the prescription and prior authorization correctly — and many clinics haven't seen the program yet. Check your criteria free (about 3 minutes), and walk in with a doctor-ready packet.

How the math works

Under the Bridge, a covered one-month supply is $50, per CMS — and that $50 does not count toward your Part D deductible or the $2,100 annual out-of-pocket cap. The calculator multiplies the difference between your current monthly cost and $50 by the number of monthly fills remaining between today and December 31, 2027, when CMS says the program ends. Someone paying $499/month for Wegovy in July 2026 could save roughly $8,000 over the program's full run.

Your real number depends on the price you actually pay. Manufacturer self-pay programs (LillyDirect, NovoCare) have changed prices before, compounded products vary widely by pharmacy, and some people's costs include shipping or membership fees. That's why the calculator lets you type your own amount instead of trusting our presets.

Three things the savings estimate doesn't include

Eligibility. The Bridge has published criteria — Part D enrollment, BMI pathways (35+, 30+ with certain conditions, or 27+ with certain risk conditions), and several exclusions. This calculator doesn't check any of that; the free screener does.

The prescription itself. Only Wegovy (all formulations), the Zepbound KwikPen only (not vials or single-dose pens), and Foundayo tablets are covered. If you take Zepbound vials today, your clinician would need to switch you to the KwikPen for the Bridge price to apply.

Normal medical costs. Doctor visits, labs, and anything your plan already charges for are separate from the $50 medication copay.

Already paying cash? You're exactly who this is for

Paying out of pocket through LillyDirect, NovoCare, or a compounding pharmacy does not disqualify you. Per CMS's June 2026 FAQ, the exclusion applies to people whose Part D plan paid for a GLP-1 during 2026 — self-pay doesn't count against you. If you've been absorbing $200–$500 a month, the Bridge is designed to bring you into covered, brand-name treatment at $50.

Frequently asked questions

Is $50 a month really all I pay?

For the covered medication, yes, per CMS — $50 for a one-month supply, not counting toward your deductible or the $2,100 cap. Doctor visits and labs are billed normally.

Does this calculator tell me I qualify?

No. It only estimates dollars. Eligibility depends on published CMS criteria, your clinician's judgment, and a prior authorization. The free screener shows how your answers line up with the published criteria.

I pay cash through LillyDirect or NovoCare now — am I excluded?

No. The exclusion applies to a GLP-1 paid for by your Part D plan during 2026, not to self-pay patients.

Which drugs are covered?

Wegovy (injection and tablets), Zepbound (KwikPen only), and Foundayo (tablets), for weight reduction and maintenance. Only 28- or 30-day fills.

Next step: the free screener takes about 3 minutes, shows how your answers compare to the published criteria, and writes your doctor message for you. No sign-up required.
